LifeLabs customers can request confirmation their information was accessed during the cyber-attack

In December 2019, LifeLabs announced that it had identified a cyber-attack involving unauthorized access to its computer systems with customer information that could include name, address, email, logins, passwords, date of birth, health card numbers, gender, phone numbers, password security questions and lab test results

LifeLabs has created a secure online form for customers to request confirmation that their information was accessed during the cyber-attack. Please use the form below to submit your request. To submit your request over the phone, please contact our customer care line.

Once the form is submitted, the LifeLabs privacy office will send the requested information within 30 business days.
