We are pleased to announce the launch of a new online form designed for creating and updating client files. Effective July 14, 2025, all Healthcare Providers are encouraged to utilize this online form when submitting new or updated client information.
To accommodate varying preferences, an updated national paper form has also been developed for those who prefer to submit via fax. Both the online form and the printable paper version will be accessible on LifeLabs.com starting July 14th.
These new submission options are intended to streamline the process and ensure timely delivery of information to our client file teams, facilitating more efficient client file creation and updates.
