How to order a replacement kit?

 Note:  If there is any issue with your COVID-19 Home Collection kit, you received an indeterminate result or if you collected your sample without being observed by a LifeLabs technician and you require a new kit, select any ONE of the options to order a replacement kit.

Option 1: Through order details page or the webpage

  1. Find the Order Confirmation email you received when you first registered your kit online.
  2. Click on the Link in the email to download your requisition.
  3. Log in using the password you created during registration.
  4. Click on the “Reorder” button beside the kit you need to replace.
  5. Confirm the delivery address.

Option 2: Phone In

If you cannot find your registration email, you can coordinate the shipping of a replacement kit by following the steps below:

  1. Contact our LifeLabs Customer Support Centre at +1 877-313-4982.
  2. Provide your Last Name and Order ID.
  3. Confirm the delivery address.

You will receive a confirmation email with details about your replacement kit shipping status.
