Kits Order Forms
The Fecal Immunochemical Test (FIT) detects hidden blood in stool, an early sign of colorectal cancer. Healthcare providers (HCPs) can order FIT kits, which include everything patients need to collect a stool sample at home and return it for lab analysis. Test results are sent directly to the ordering HCP.
FIT kits are available for HCPs across Canada except British Columbia. There is no cost for HCPs to order the kits. The kit is an alternative for patients not covered by the Ontario Health’s ColonCancerCheck program. The $75 cost is billed directly to the patient when the kit is returned to LifeLabs.

The Vitamin D At-Home Test measures vitamin D levels using a simple dried blood spot (DBS) sample. Healthcare providers (HCPs) can order kits for their patients, who collect a small blood sample at home and return it by mail for lab testing. Results are sent directly to the ordering HCP.
Vitamin D kits are available for HCPs across Canada except British Columbia. There is no cost for HCPs to order the kits. The $89 test fee is billed directly to the patient when the sample is returned to LifeLabs. This private pay option is ideal for patients not eligible for public coverage and seeking convenient, at-home access to vitamin D testing
The HbA1c At-Home Test provides an assessment of a patient’s average blood sugar levels over the past three months using a dried blood spot (DBS) sample. This convenient, at-home collection option is designed to support remote or hard-to-reach patient populations. Patients collect their sample and return it by mail for lab testing. Results are sent directly to the ordering healthcare provider (HCP).
The HbA1c At-Home Test offered exclusively to HCPs with an existing contract. HCPs interested in accessing this test for their patients must contact us through www.lifelabs.com/contact-us to explore eligibility and partnership options.
The HPV test screens for high-risk strains of the Human Papillomavirus, linked to cervical cancer. HCPs will soon be able to order HPV self-collection kits, allowing patients to collect samples at home for lab analysis. Results are sent directly to the HCP. (Coming soon).
