We’re pleased to share with you the launch of a free Health Assessment tool in LifeLabs’ MyCareCompass online health portal, available to Ontario consumers. Patients can access the Health Assessment through their MyCareCompass account, and it takes about 10-15 minutes to complete a short list of questions that focus on 4 key areas of health: About Me, Lifestyle, Mental Wellness, Health History. The Health Assessment will provide patients with insights into their health and a personalized report with steps they can take towards improved lifestyle choices in their journey towards better health
Key points to note:
Benefits: The Health Assessment uses a patient’s self-reported information to provide them with insight around their health habits / lifestyle to help them make informed decisions around their health journey and wellness. By offering insights into their health, habits, and behaviours, it may help create awareness on healthy habits and steps they can take to improve their wellbeing. It can also be used to begin a dialogue with health care providers on addressing health concerns and/or health goals.
Creation of questions: The Health Assessment was created based on current Canadian guidelines, health literature and validated health tools such as the Framingham Risk score and GAAD 7. It is updated regularly as new guidelines, recommendations and literature become available.
The Health Assessment was built in collaboration with Core Health (now Carebook Technologies), a global leader in health solutions, with more than 20 years of experience in the digital health space.
Sharing Health Assessment reports with a health care provider: Patients are able to share their Health Assessment report with their health care provider by downloading a PDF copy of the assessment. Or, they can take a printout of the results the next time they plan to see a health care provider and want to discuss making changes to how they manage their health. If they do not have a health care provider, they can connect with one of the virtual health care partners through the MyCareCompass platform.
Frequency of completing the assessment: It is recommended to complete the assessment if a patient has made a significant change in their lifestyle, or if aspects of their health have changed significantly.
Accessing the Health Assessment: The Health Assessment is accessible to Ontario customers over 18 years of age with their own MyCareCompass account.
